Only Orc characters have immediate access to the strongholds unchallenged. Characters of any other race must first become Blood-Kin of the Orcs before being welcome in the strongholds. You can enter a stronghold without being Blood-Kin, but everyone will continuously tell you that you are not welcome, and you are unable to trade with any merchants in the stronghold.

Completing any of these quests will have the grateful Orc say, "For your help, I'm sending word to the Orc strongholds that you're to be trusted. You should be welcome there now.", signifying that you are now Blood-Kin.
There are four Orc Strongholds in Skyrim, and except for one location that connects to a Daedric Quest, the other three strongholds each have the same method for gaining entrance. If you choose to play as an Orc during character creation, then you can enter every Orc Stronghold without completing a task.
Dragons can spawn in many locations, but if you happen to see one near an Orc Stronghold, then kill it for easy access into the Orc Strongholds of Skyrim. You can't guarantee that a dragon will spawn near any of the strongholds, so this method works best if you happen to get lucky.
Orcs here live according to the rule of Malacath. Malacath, as Daedric Prince of the Orcs, is worshipped in these places. The strongholds are for Orcs only, but the Dragonborn can be granted entry if they complete any of the qualifying tasks. If the Dragonborn is an Orc, access will be granted automatically.
In the middle of the Merethic Era, the Orsimer and their leader Trinimac attempted to halt the movement of the prophet Veloth and the Chimer. The Daedric Prince Boethiah ate Trinimac, corrupting his body and spirit, and he emerged as the Daedric Prince Malacath. All of the Orsimer people were also changed into what they are now known as, the Orcs. The early Orcish people founded the tribal and independent lifestyle of living. Instead of claiming one area, they would settle in multiple provinces dependent on the preferences and lifestyles of each individual tribe. These provinces came to be known as High Rock, Skyrim and Hammerfell. The Orcs would form small, tightly-knit strongholds and villages across these provinces while others gradually contributed to the formation of the predominantly Orcish city of Orsinium.
During the early Fourth Era and the sacking of Orsinium, many surviving Orcs fled east into Skyrim while others fled into the Dragontail Mountains or the Wrothgarian Mountains and successfully integrated into the clanish strongholds.
Strongholds are governed by a rigid class system. Every Orc has a specific job and is obligated to perform it. Every man, woman, and child in the fort is also trained from birth to defend it. The strongholds are led by a chief, who killed the previous chief to prove his strength. Only the chiefs are allowed to have wives, and due to this, they often have several. This method is to ensure that the strongest Orc becomes chief, contributing his genes to the next generation of Orcs who, in theory, should be stronger than the last.
Orcs in strongholds follow their own law called "The Code of Malacath." The will of Malacath is clarified by the stronghold's resident wise woman. The rules of the Code are simple: don't kill, don't steal and don't attack people without reason. They do not have jails; instead, offenders are forced to pay a "blood price," in which the criminal bleeds until the victim is satisfied. In cases where the victim was murdered, the chief is consulted to impose an appropriate penalty. All Orcs in the strongholds belong to a faction called Tribal Orcs. Committing a crime against any of the Orcs in any of the strongholds results in a bounty in all the strongholds.
If the Dragonborn is an Orc, they will be welcomed by the strongholds of Skyrim, upon approaching one of the strongholds, the Orc watching the gate will speak to them, starting the following conversation:
Asking a guard at one of the strongholds if there are any others will reveal all the other strongholds on the map as unexplored locations. Some city hold guards might talk about one of the strongholds if the stronghold is in the same hold.
Among the many races that inhabit the land of Tamriel in The Elder Scrolls V: Skyrim, the Orsmier (or Orcs) are some of the most segregated. Their strongholds are dotted around the continents, and each contains its own society with a rigid class system. Orcs within these strongholds follow the teachings and laws of the Daedric Prince Malacath, who is the progenitor of all Orcs in Tamriel.
Skyrim players can encounter and enter several Orc strongholds, though their residents will (by and large) be unhappy to see an outsider trespassing on their land. Players who choose to create an Orc will automatically be granted access to all the strongholds, but it's possible for non-Orc players to gain the trust of the strongholds -- and doing so provides some interesting rewards.
There are a variety of ways to gain passage to and from the various strongholds, each of which involves completing a quest or task for an Orsimer resident of Skyrim. While many of these are easy to obtain and complete by chatting Orc citizens, even easier is to approach the gate and be confronted by a guard. The guard may task the player with locating the Forgemasters Fingers, and acquiring the requested item will allow the Dragonborn peaceful entry to all strongholds.
Due to the limited number of Orc Strongholds (and the race's relative rarity), players can reasonably finish entire playthroughs without coming into contact with these settlements. However, they should make an effort to locate at least one of these strongholds -- not least of all for the associated Daedric Quest at Largashbur.
Players can ask certain residents at strongholds if there are any more located in Skyrim. Doing so will mark any undiscovered ones on the map, making it simple to visit them all after locating one. These secretive societies are well-worth a visit from any player, providing an interesting look at the inner workings of the Orsimer way of life.
No pest has proven more resilient to the Nords of Skyrim than the common Orc. The tusked people claim occupancy of our fair realm, stretching back before the time Ysgamor crossed the Sea of Ghosts. Though few written accounts still exist of that time, there is mention in the histories of the Companions uprooting Orc strongholds even as they burned the Snow Elves from the land.
Orc strongholds are well defended outposts often situated around deposits of mineral wealth. Each contains the infrastructure to support a dozen or more families, surrounded by walls designed to repel anything but the strongest martial force. Many jarls are remembered in song for dying vainly while attempting to root a stronghold from their lands. Worse, destroyed strongholds often return within a generation of their destruction if not countered by a Nord fort that needs a constant supply of troops and provisions.
